    This ?

    in D17

    =INDEX($E$3:$M$12,MATCH($B17,$B$3:$B$12,0),ROWS($1:1))

    in E17

    =INDEX($N$3:$N$11,MATCH($B17,$B$3:$B$11,0))*$D17

    in F17

    =INDEX($AD$3:$AL$12,MATCH($B17,$B$3:$B$12,0),ROWS($1:1))

It is better to include few (3 or 4) sheets from which you are taking data for calculation.
    Pl note
Array formula should be confirmed with Ctrl+Shift+Enter keys together.
If answere is satisfactory press * to add reputation.
