[Power Query] Unpivot Columns and then Append them

Hi All,

Need your help on a task which seems simple but it has driven me crazy (beginner in PowerQuery here).

I need to change the first format to the second format as shown in the attached example sheet. The challenge I have is that the data is scattered across columns so the straight forward "unpivot" did not give what I needed.

What do you suggest?
