Array that returns multiple results matching a date

In the "Apr" tab, in cell D37, I have the date April 12, 2022. In the "Crop List" tab, I have a table called "Crops". I'm trying to create a formula that matches the date in cell D37 to every instance of that date in columns D and F in the "Crops" tables. If the date is in column D, I want it to return the value in column G. If the date is in column F, I want it to return the value in column H. I've used INDEX, AGGREGATE, MATCH, combinations of those, etc. Still can't get it to work. Any suggestions or directions?April tab.PNGCrop List tab and Crops table.PNG
