Need help reformatting data (possibly expand & rotate?)

Hello!

Having a little bit of trouble creating a formula and hoping someone here can help.

In the sheet attached below I have data in cells A1:T4. Below that you will see a table already made based off the above data in cells A1:T4. As you can see the table is reformatted to where it goes vertically and each name listed is typed out based on how many numbers are in row 1.

My question is, what formula would I type to recreate the table shown at the bottom? Some one made that table and formatted that way for me but did not provide a way to actually do it for other data sets.

Any help?
