Percentile If with Multiple Conditions

Hi - I am trying to write a conditional percentile formula with multiple conditions from data in a table. See attached mock-up sheet for the problem I'm having. When I write the formula using Percentile.EXC, I get a NUM error, and when I write it using Percentile or Percentile.INC, the formula returns 0.

Note that when I use this exact methodology, but without the AND() and only with multiple condition, it works perfectly.
