Index Match fomula, How to remove duplicates ?

Hello every one

I have two sheets (attached), first one I have the data and in the second one I want my data come automatically.

I want the Taxpayer name with 2 % tax (yellow in sheet1) into Taxpayer Name column (yellow in sheet2). I have already used the Index and Match formual but it will give me the first name only.
How to have all names with 2 % tax in row 17 to 26 (sheet2) ?
