Vertical and Horizontal Countif between dates with multiple criteria

Hi all

Some help would be greatly appreciated, as I am no quite sure why the formula isn't working. The formula is below, it basically should allow me do a vertical and horizontal using multiple criteria and allow me to check between date ranges. It has worked fine for one aspect of what I am looking at but when I added an additional criteria it wouldn't work.

When the additional criteria isn't in, the formula is fine and works. However, I am not sure why the addition causes an issue.

Could it possibly be that I am using the same range as I am using on the dates? the issue is that the dates and the 'Yes' column noted above are next to each other. So for example, I have A is a date column with B is being a column where my 'Yes' could be found then C is date again with D being a 'Yes' column and so on and so on.

I hope all this makes sense. I'd be grateful for any help.
