Retrieving multiple text strings based on multiple criteria (formula update)

If anyone can assist me that would be great.

Summary
The enclosed file has an old desired result to extract certain information. Currently there are ID's (Column A) that withdraw FROM (Column C to E) a program TO (Column F to H) another program. More than one withdrawal can occur in any year (Column B), hence why there is three columns for FROM and TO, as there can be up to three withdrawals in any year.
