Complex Index Match trouble

Hello,

I've attached a working example of my issue. By changing the MFG Date in Column B, the EOL % recalculates in Column D based on data in Sheet 2, Column E (type field), and the Table beginning on line 10. Lines 3 & 4 are an example showing one I have working, but for some reason now I am struggling to add in a "third criteria" (i.e. if the Column E field = "12V") I need it to operate just like my existing lines do for Column E type of "V" and "F".
