Help adding countif to a formula

Hey There,

I've attached a sheet with sample data and desired result. If I can get some help adding countif to remove the duplicates from results row that would be great! Or if there's another formula(s) that work better that's great too.

Formula is: =INDEX($F$6:$F$35,(MATCH(1,($B$6:$B$35>=$I$5)*($B$6:$B$35<=$J$5)*($C$6:$C$35=$H$5),0))) entered with CSE

You would need a different array formula:
Enter formula in H8 and drag across

***Array formula
...confirmed by pressing CTRL+SHIFT+ENTER to activate the array, not just ENTER.
You will know the array is active when you see curly braces { } appear around your formula.
If you do not CTRL+SHIFT+ENTER you will get an error or a clearly incorrect answer.

Works perfect, thanks so much. I just could not figure it out, I could either have multiple criteria with duplicates or one criteria and no duplicates.
