I agree that the form is clunky, but it is not for me.
I was asked to create something with individual sheets where the user can either enter the information directly into the individual sheets, and add images if required, or print our the document to complete manually during a meeting - the information is for record keeping and not data analysis. In all probability, the individual sheets will be printed out and put in a file - though I am trying to break them of this habit!
