Index and Match formula not returning correct values

I have two workbooks. In column V of the Coastal workbook, I want it to return whether the customer is a Dealer or Grower from Copy of Dow 2 workbook, column D. My formula is all out of whack and I do not know how to fix it. Furthermore, if by doing the index and match it finds that the customer is a dealer I want it to put a ST in column V of the Coastal workbook and if the customer is a grower I want it to put an EN. I hope someone can please help me. Many thanks in advance!

In what way is your "formula is all out of whack"? The formula appears to work OK, It obviously returns #N/A if the name is not present.
