Filter is long , I know that , you have to unclear filter for next query , query work is common so formula is more practical
Store 2 excluded , as this is sample data , real data has 22 stores
Then add in your file, what your input data are (and in which cells, we can find them).
You could mark them with a color (e.g. blue).
Blue is the search criteria formula on n2 to y2 to drag down
a2 = B2&countif($B$2:$B2,$B2)
P2 =VLookup($O$1&Row()-1,$A$2:$M$17,column()-14,0) and drag across

I don't see 11 and 12 in the file in #9, so the question is unclear to me.
You get better help adding an excel file without confidential information.
Please also add the expected result manualy in the file.
I have not added column 11 and 12 , the formula then not works
If you are inserting two more columns (J & K) then change 14 in the formulae you have been given to 16
=VLookup($O$1&Row()-1,$A$2:$M$17,column()-16,0)
