Index match multiple criterias

Hi everyone,

Thank you for taking the time to read this post, I appreciate any feedback that will guide me into the right direction.
I am attaching a spreadsheet, based on multiple criterias probably using index/match , but only wish to insert a score and hope this will give a corresponding grade. Please see the sheet for an example.
