Combine Multiple Columns (More than 2)

I need to combine data from multiple columns into one column. On my spreadsheet, a column was created for each church or organization with whom we work. Generally only two of these columns apply to each individual.

On each row are individuals. I would like to combine the columns so that I only have one column for TEAM and one column for DATE.

NOTE: The attached spreadsheet only contains the first 10 rows. I'd like to be able to expand this to an entire spreadsheet that ranges from Column A to Column DZ.

What would you like the output to look like. I am little dense and not quite sure what you are looking for. Post an example of what the results should like like.

In cell K2 array-enter this formula and fill down to K10

If you aren’t familiar with array-entered formulas array enter means the formula must be committed by simultaneously pressing and holding down Ctrl and Shift while hitting Enter.
Then array-enter this formula in L2 and fill down to L10
    Formula: copy to clipboard

The workbook is attached and produces these results.
