Help require to create a signature

Hi friends,
There is a list of teachers on ‘Information’ sheet.
I want a signature on ‘Card’ sheet on the following conditions.
If the name of teacher has a title/prefix ‘Mr.’ then there will be a signature starting with ‘Sir’
Else
The signature will start with ‘Madam’
