I am working on a "Progressive Disciplinary Chart" for my workers at my job site. Basically it will allow us to mark down how many times a worker breaks a specific rule and then allows us to see if he broke that same rule in the past, how many times he has broke it and what type of disciplinary procedure he should get next. All of that I have already completed.

However, I am a nerd and want it to be super user friendly and easy to use because it will be other leaders at my site who are also adding points and for CYA purposes, I'd like it to detail who it was who entered it in and clear to them that they did it correctly.

So for starters:

To give a point to a worker, the leader will select the infraction that is next and a drop down will appear.
- The drop down will have one selection "Give Point"
- After they select "Give Point", I want it to produce a pop up box telling them to sign their name. This will allow me to see who it is who gave the point.
- After they sign it and point is given, I want the text to say "Point Given" and then if you hover over it, it says who it was who gave the point.


I have included a sample page if this is too much to include as a set of instructions for me to do myself so anyone willing to help can just do it for me and I'll learn off their example.
