Need formula to count score based on criteria (0/1 variable, time series)

Hi Forum

Hope someone is able to provide a formula I can use to calculate a score based on specific criterias. The data base is sorted as a 0/1 - Ok/Failed - variable over time. Following criterias must be in place.

Level:
a) 1
b) 2
c) 3
d) 4

Criteria (A):
a) 1 month failed
b) 3 months in a row failed
c) 4 out of 6 months failed
d) 6 out of 9 months failed

Criteria (B)
a) Stay in level 1 0 months after OK
b) Stay in level 2 1 months after OK
c) Stay in level 3 2 months after OK
d) Stay in level 4 3 months after OK
