how to unpivot

I have a table with results from pivot. I don't have the original file now. I would like to unpivot it and would like to know the formula. I have attached the excel file to show exact example and result I would like to have. I googled and also tried using the HELP in EXCEL but unsuccessful. Any help would be much appreciated.

I don't have the values as in the link. However, I tried and getting the result as shown in the image which is not as excepted..

I would like to have the App Name for each person.
