CHALLENGE: Extracting data from complex Excel Forms

Hi all,

I have been trying to extract specific information from an excel form.

The data is particularly unfriendly to manipulate and I have been struggling to find a solution to extract the form data into a listed table.

You will be able to see in the attached sample spreadsheet that on Sheet one are two examples of the extracted forms (these total in the hundreds) and on Sheet two are the headings I wish to extract.

I am struggling with looking up the headings from Sheet 2 within Sheet 1 and returning the data in the specific cells underneath.

I've experimented with lookups and match/indexes but to no avail.

Any help would be great.
