IF(AND) formula with dates working for some months/years but not others

I'm using a pretty simple IF(AND) formula to produce a code; it should show 1 if a date falls within a range of dates, or 0 if it does not. I have multiple months for multiple years. For some reason, the formula works great for every month in each year, except for September 2013. I've attached my spreadsheet and have highlight the cells where the formula should be producing 0's but is incorrectly producing 1's. Any help would be appreciated!

because you put it as Sep 31 but Sep has only 30 days

change cell O3 to Sep 30
