Formula needed for partial string match as part of index match with multiple criteria

Gents,

I have spent about 7 hours trying to insert the Left function into multiple criteria Index Match formula's that I have found on the internet but have been unsuccessful. Here is what I am attempting to do:

Here is the best I could do at putting a formula together:

=INDEX(ControlsPricing,MATCH(1,(MODEL=MATCH("*"&LEFT((ModlNumber,5)*(Motor=MotorType),0),ControlsPricing),0)))

Anybody out there have a solution? I have also attached my example worksheet.

I had to use bogus data so I did have some errors in my example data - thanks for pointing that out.
