Match Multiple Values with Multiple Values from Another Table - Return a Single Value

In cell C4 try this:

=INDEX($B$24:$J$33,MATCH($B4,$B$24:$B$33,0),MATCH(YEAR(C$3),$B$24:$J$24,0))

Autofill across/down as required.
