Database headers as Row objects in Pivot Table

Hi

Am not that good with Pivot tables ... actually not good in excel at that.

I have existing data in a database and I would like to report the data header as row objects in excel (see attachment). Am not sure it's even possible, but help if you can.

Tab 1, has original data
Tab 2, what I would like as output
