Multiple Columns in 'Multiple Consolidation Ranges' Pivot Table

I have a tab in my spreadsheet that is a matrix-style data entry page, and I need to output it into a pivot table. I have it almost correct, but I can't seem to get the second column criteria to display properly. It just says 'Data'.

I have a list of cost codes down column A (column A on 'Cost Code Matrix' tab). I have the week numbers across row 4 (row 5 on 'Cost Code Matrix' tab). What I need to get but can't is the date corresponding to the week number showing as a sub-column in the pivot row 5 (row 6 on 'Cost Code Matrix' tab).

I'm obviously missing something here, but this style of pivot table is not my forte. I don't like collecting data from matrices.
