Some help with Pivot table & Chart

An old work colleague sent up an excel document for me which I have attached, I have had to make some changes but I have missed something an as I am no longer in contact with said colleague am unable to ask him for help.

Would someone be as kind to have a look and let me know what i need to do to get this working correctly

The bits that aren't working are

I can't seem to figure out where I need to amend it
