I had attached my worksheet in which "original" sheet refers to the data available & the "required format" sheet refers to the output required for me.

In B2 and copy right and down,
=INDEX(Original!$B$2:$C$28, MATCH('Required format'!$A2, Original!$A$2:$A$28, 0), MATCH('Required format'!B$1, Original!$C$2:$C$28, 0))
