2016 Chart Data Table Issue

Hello,

I recently upgraded to 2016 from 2010 at work. I'm using a document I created in 2010 and am running into several issues I have listed below.

1. Chart - Data Table Headers

*Please note several headers are missing and the initial one is incorrect

2. Chart - Data Table Alignment

*Please note that the bars do not match up with the data table

Troubleshooting already completed:

1. Re-selecting chart data
2. Selecting "none" for the headers (displays Jan-00)
3. Turning off legends (both in data table and in chart)

Any help would be much appreciated!
