Pivot Table Layout

Hello everyone

I am trying to set up a pivot table for the attached data but am running into difficulties obtaining the format I need.

I believe my problem is because I am referencing the cell header date in my formula so my table may need rearranging but I am not sure how is best.

I have tried to explain what I require in the attachment so will not go into detail here but will do if asked.
