Hyperlink to Map

I am trying to create a hyperlink to map an address in my database. The fields are Address1, City, State, Zip. I am using the hyperlink builder to create. This is the path I am using;

https://maps.google.com/maps?q=[Address1]+[City]+[State]+[Zip]

When I click on the link it will go to the google map but in the search address it shows [Address1] [City] [State] [Zip]. If I type in the actual address in the brackets it the map will locate. Problem is I have 350 addresses and don't want to type in each individual address. any help would be greatly appreciated.

I am using Office 2013.

Thanks for the response, but not sure what you are showing me here. My database is in Access, which I failed to mention, sorry, and when I click on the "Map" hyperlink I want an to go to google maps showing the destination.
