Iissue Creating Data Base. Please Help!!

I have issue crating a data base and Query. every time i select Name, DOB option button and hit Enter Data. the data is not getting saved for the Party ID.

Please if someone can help me out set it up I like if I select the Name, DOB, or any other option button is selected that it will show in the data base for the Party ID if i hit Enter Data and for the Enter Data button needs to refreshes for me so i can input the next Party ID,
