Create a Report

Thank you for your reply. I check the video and it's not quite what I am looking for. My combo box allows a user to select the values to search for and will display the results in a the list box. I want to give the option to the user to select multiple items in the list box and then create a report out that shows what is in the combo box and what the user has selected in the list box. I have attached an example of what I want the report to look like after the Preview/Report button is clicked.
