Macro to create form by pressing button

I have a query that when I Click a button I should have a data from another source added to a specific table with in specific value?".....can you please help me

I am attaching a sample excel where in when I select a check-box in sheet1(Emp ID) the form in the payslip should be fill automatically. can any on please help me this function.
