    HTML or a Web Form using Excel as the database....

    Hi All,

    I知 requesting for help to Create a Web / HTML Form.

    1. I get a CSV / XLSX file daily which has some data (Demographics part of the attachment)
    2. Request is to be able to upload this file on an interface. Each entry can be filtered by using the unique code in each line using a dropdown menu
    3. After filtering I値l need to deal with each entry individually my team needs to plug in the feedback (The questions in the attached Feedback form should appear on the web form similar to a survey link / feedback form)
    4. Once the data is fed I should be able to extract the complete dump in the form of a CSV / XLSX file with details

    I知 not sure if this can be done, and if this is the right place to post this query.

    I値l appreciate if someone could help me create this or direct me to the right path.

    I don't have access to my system currently, which is why I haven't been able to see what it exactly looks likes.

    What I'm actlually looking for is a weblink or similar where I can upload a file with Demographic details, and my team can then use it to provoide feedback,with the goal of getting it back in an CSV of XLS format. The data needs to get cumumative at the backend with giving us flexibility to retrive back in excel.

    I just saw the awesome piece of work you've shared with me it is phenomenal. I'm absolutely amazed with the type of work you all do here and help keen learners and xl enthusiasts like me learn and help solve our problems. Much appreciative of everything is all I can say....

    Thanks for the compliment but I have to say the questionnaire generator wasn't my piece of work.. It was uploaded to the forum by somebody who's name eludes me now.. I thought it may fit the bill for what you were looking for so there's no point in me re-inventing the wheel.
    As for web based stuff, I really can't help you there I'm afraid.

    No worries! Your interest in helping in one way or the other keeps our motivation going!

    Do you think a web form like the one I am looking for can be done without a database as such i.e. using excel 2007 or 2010?

    Actually, am I seeking the help in the right direction is what I want to know...

    I'll also try and embed my questions in what you've shared to see if it helps.... I'm assuming it is dynamic enough in terms of adding more questions etc...The only concern is about pre-loading the Demographics in this questionnaire (my first part in the doc I shared in the first post).

    I'm pretty sure it's possible to create a questionnaire in Excel and upload it to the web but don't quote me LOL....

    No it isn't, at least not reliably - Excel is not a web medium.

    Why not have a look at google forms?

    Hi All,

    Google Forms was pretty easy though not a complete solution to my problem!

    While I research to get to what I'm looking for. Could any one advise if there is a way to add a CSV / XL file with some pre-populated data that can be filtered on the Google Form itself depending upon the criteria on which the form is to be filled like: Date, Name Branch, etc..

    Ideally, I would like to upload a CSV file with some Demographics etc. That need to be filtered before filling the form. Is it possible with Google Forms? I need to do this daily, and the explanation and provision here does not cater to what I need, unless I'm missing anything....

    Pre-populate form answers

    While working on your form, click the Responses menu, then select Get pre-filled URL.
    Fill in any answer fields you'd like to pre-populate.
